HAIR TRANSPLANTATION
What are the causes of hair loss?
There are many reasons for hair loss in men and women. However, the most common cause in men is male pattern hair loss with familial inheritance.
Dihydrotestosterone (DHT), the hormone responsible for male pattern hair loss, causes hair loss in genetically predisposed patients.
Similarly, female pattern hair loss in women shows a familial transmission, but this is at the research stage. Hormonal effects in familial hair loss in women differ from men.
Less common causes of hair loss that should be investigated may be due to thyroid diseases, iron deficiency, high fever, very aggressive diets, childbirth and medications taken. However, dermatological diseases related to the scalp can cause temporary or permanent hair loss. These diseases can be counted as lupus, lichen planoplaris and alopecia areata.
There are many rumors circulating among the people about hair loss. These include poor circulation, frequent shampooing, wearing hats and helmets.
It should be kept in mind that every adult loses 75-125 hairs a day because of the hair in the dormant phase (telogen). Hair follicles that come out of this phase with shedding begin to grow as new hair strands (anagen phase). This process remains in a certain balance and the number of hairs on the scalp remains constant.
Stress, another minor factor in hair loss, can increase male pattern hair loss.
When you become aware of hair loss, you can reverse this situation if you wish. One of the options against hair loss is drug treatments, which we call medical treatments. Drug treatments aim to stop hair loss and stimulate hair growth.
The fact that the drug to be chosen in hair restoration treatment has been approved by the FDA (American Food and Drug Administration) is a situation that should be questioned in people who are facing the problem of hair loss.
Finasteride (Propecia) is a drug that has been widely used in the last decade and is taken orally in tablet form. Scientific studies have shown that Finasteride (Propecia) stops hair loss in most men and stimulates hair growth. Finasteride works by reducing the amount of DHT (dihydrotestosterone). This hormone is largely responsible for male pattern hair loss. At the same time, finasteride (Propecia) does not affect the testosterone level. Finasteride (Propecia) is most effective in mild to moderate hair loss. However, it has the feature of being the most recommended drug by the hair surgeon as an adjunctive treatment to the patient undergoing hair transplantation.
Topical minoxidil (Rogain=Minoxil) has been a treatment to help slow hair loss for more than 15 years, but is much less effective for new hair growth. The 5% concentration of minoxidil is recommended for men. For women, a 2% concentration is recommended as this concentration stimulates facial hair growth.
In order for both treatments to be successful, continuous application is recommended. If the treatment is not continued, the hair loss process recurs. Although it is a very assertive statement to say that these treatments are effective for everyone, the treatment is very successful. Using both drugs together may provide more benefits in male pattern hair loss.
In the most basic sense, hair transplantation is to take hair follicles from the back of the head, from the area between the two ears, under the skin, with the genetics of not falling out, and transfer them under the skin to the areas where we want to have hair on the front.
One of the most striking points at first glance is our hair. Our hair is different in quality, density, color, length, and we all do our best to make our hair look different and beautiful with different shapes and combing.
The color, shape, shortness or length of our hair gives clues about our stance, style, taste and likes in life. While the presence of our hair is associated with youth, its absence causes us to be perceived as older than we are.
The person who encounters hair loss feels socially and culturally incomplete and loses self-confidence. This negative perception negatively affects individuals in all areas of life. This is exactly why our hair is important to us.
Despite all the negative effects on individuals, 50% of 50-year-old men face the problem of hair loss. In order to get rid of this situation, individuals try many different treatment options, mostly over the years. Unfortunately, the regrowth of hair lost with medical treatments cannot be achieved with medical treatments, and the most effective treatment of hair loss in our modern world is hair transplantation.
History of hair transplant techniques
The adventure of hair transplantation started in the 1940s. With the development of the Follicular Unit Technique in 1998, natural, aesthetic looking hair transplantations were started and the foundations of modern hair transplantation were laid. In 2003, the FUE method, in which the hair follicles are taken one by one, was defined and determined as the gold standard technique for hair transplantation.
Modern hair transplant operations
Hair transplantation operation is briefly taking hair follicles from the back of the head and transferring them to the areas where hair loss occurs. It can also be called a kind of displacement operation.
The hair on the back of the head is the hair that we know as donor dominant, which has the capacity to grow for a lifetime. When these hair follicles are transferred to bald areas, they do not lose their hair growth abilities. The dominant donor field is the scientific basis of hair transplantation.
Every individual with sufficient donor hair follicles at the back of the head is a suitable candidate for transplantation. While many patients who had hair loss in the past were not suitable candidates for hair transplantation, with the development of modern techniques and the perception of hair transplantation as an art, almost everyone who has hair loss problems can have hair transplantation today.
Today, what is desired to be achieved with FUE hair transplantation is not only the growth of the transplanted roots, but also to obtain a natural-looking result. No one wants to be noticed that they have had their hair transplanted.
In the past few years, great progress has been made in hair transplant surgery, and applications that result in patchy, cobblestone, grass man appearance have been replaced by single-haired root transplantation, micro and mini graft applications.
The use of gentler, finer roots, advanced and new surgical instruments have made it easier for hair transplant surgeons to perform personalized, natural-looking, successful hair transplants.
With the use of single-haired roots, naturalness and soft transition in the hairline are achieved. Creating a new hairline always requires surgical experience and artistic skills. When a person who does not experience hair loss is examined, it can be easily noticed that the hairline consists of very thin, gentle hair, does not continue like a straight line, and the hair follicles thicken from the back to the front.
The side effects of modern hair transplants are a slight pain and discomfort after the operation, a slight swelling around the eyes, and red dots and crusts that occur in the recipient area for up to a week. Bleeding, infection, scarring are seen so little that there is no scar. Modern hair transplant surgery is a comfortable surgical operation with predictable results and satisfactory results for the patient.
Hair loss is a lifelong process. The continuation of hair loss or the desire to achieve more density is possible with repetitive hair transplantation. With modern hair transplantation techniques, a large number of hair follicles are obtained in one session, so fewer operations are performed to achieve the desired result.
Hair transplantation is a permanent procedure. Hair follicles transplanted during transplantation are genetically resistant to the hair loss effect of testosterone. The transplanted hair continues to stay in the area where it was transferred for life.
The units (follicular unit) transplanted during hair transplantation are hair follicles. Hair follicles (follicular unit) are anatomical structures that contain single, double, triple or more hair strands. Since hair follicles (follicular units) are structures with anatomical integrity, the use of hair follicles during transplantation ensures that the result is natural, aesthetic and productive.
Hair follicles (follicular unit) are taken from the area between the two ears and the nape, where the hair is particularly dense and resistant to shedding. Thus, the transplanted hair does not fall out and the result is permanent after hair transplantation.
The amount to be transplanted in hair transplantation refers to the number of hair follicles. If we talk about hair strands, the number of hair strands can be more than twice the amount of hair follicles. In this sense, it can be confusing to talk about the number of hairs recommended to the patient before hair transplantation. Considering that each hair follicle (follicular unit) contains more than one hair strand, when we transplant 3000 hair follicles, we actually transplant 8000-10000 hair strands.
FUE HAIR TRANSPLANT
Fue Hair Transplantation is a minimally invasive and absolutely advanced hair transplant technique.
In the Follicular Unit Extraction ( FUE ) method, the hair follicles in the donor area are removed without sutures, thanks to 07-09 mm surgical equipment.
In this way, a result that can be called traceless is obtained in the donor area after the FUE operation.
Follicular Unit Extraction ( FUE ) is considered a very long procedure compared to other Hair Transplantation techniques.
After Follicular Unit Extraction (FUE), the level of pain will be extremely low compared to other operation techniques.
FUE operation experience of the Specialist Plastic Surgeon in Follicular Unit Extraction ( FUE ) is the most important criterion for the success of the operation.
FUE Hair Transplant- 15 days before the operation
If you are taking any blood thinners or beta-blockers, you should share this information with your doctor as soon as possible.
Never cut your hair short in the two weeks before the procedure.
FUE Hair Transplant - 7 days before the operation
Before FUE Hair Transplantation, which is a minimally invasive procedure, you should stop the intake of B or E vitamins, which may increase the amount of possible bleeding.
You can continue these vitamins within three to five days following the operation. You should not take aspirin or any other anti-inflammatory medication for 7 days before the operation. Medicines can be used three to five days after the operation.
You regularly use; You can continue to take heart and blood pressure medications in your daily routine.
It would be the right decision to quit smoking during this period, as it negatively affects and delays wound healing after the operation.
FUE Hair Transplant – Operation Day
Take a shower on the morning of the operation and wash your hair as usual with your daily shampoo. Do not use spray, gel, or any other styling product on your hair. Wear comfortable casual clothes rather than the clothes you would have to wear over your head. It is preferred that you do not use heavily scented cologne or perfume on the day of the operation. Depending on your operation time, you can have a light breakfast or lunch. Reduce your daily coffee consumption. High caffeine consumption can increase bleeding and sensitivity. On the day of the operation, do not bring your valuables to the operating room.
Your doctor briefly reviews the notes taken during the previous examination and answers any questions you may still have.
During the final examination, he will describe and draw the planned operation area to you again.
If the last examination was done more than three months ago or if there is a physical change in your head area, your doctor may suggest a new planning based on the current situation.
After the examination, the Specialist Plastic Surgeon will take your photos showing the head and face areas to add to the medical records. (In terms of patient confidentiality, these photos are not shared with third parties without your written consent.)
After the transition to the Operation Room, first of all, sedative anesthesia is applied.
You will need to lie face down in a comfortable medical chair where you can breathe easily.
After the application of local anesthesia in your head area, the effect of anesthesia is expected for 10-15 minutes.
Your doctor starts to remove the hair follicles one by one with 0.6mm-0.9mm diameter microsurgical equipment.
At this point, the diameter of the equipment to be used will vary depending on whether the hair follicle groups to be taken are single-double-triple.
Simultaneously, the procedure called graft dissection begins to be performed under the microscope by two or three nurses.
After three to four hours, your doctor will reach the planned number of grafts. This period; It varies according to the amount of graft, hair type and skin characteristics.
30 minutes After the lunch break, the 2nd stage of the procedure begins: for the placement of the collected roots, Exp. New root sockets with millimeter thickness are opened by your Plastic Surgeon.
Roots refined under the microscope, exp. 2-3 nurses are placed in the new root sockets prepared by the plastic surgeon.
At the end of 7-8 hours of operation, the recipient and donor area is finally checked again by the Specialist Plastic Surgeon.
After the operation is over, your doctor will have a Post-Op meeting with you. During this interview, post-operative instructions will be explained and a written copy will be given to you to take with you.
In this file, besides the instructions, there will be a telephone number where you can reach your doctor directly if needed.
In addition to the file, your medications will be given to you by your doctor after their prescription has been described.
Finally, a tennis band is attached to your head that covers the dressing area.
After the Hair Transplant Operation, you should sleep for a few nights by using two pillows, taking care to keep your head above body level.
If necessary for a comfortable sleep, medications for sleep and pain will help you in this regard.
In the morning following your hair transplant surgery, you are invited to have a hair wash in our clinic.
During this application, how to wash and dry your hair and head for the next 9-10 times will be explained in practice.
It is recommended that you keep alcohol and cigarette consumption to a minimum during the two weeks following your Hair Transplantation and even quit.
After the 10th day, it is possible to shape the hair with scissors. It is recommended to use scissors for 1 year in your haircuts.
After six weeks, you can dye your hair if you need it.
For the first six weeks, you should protect your head from strong sunlight.
You can start your daily exercises as of the second week.
You should not stay for a long time in similar environments such as hammam and sauna, which may increase your blood pressure.
EYEBROW TRANSPLANTATION
The main task of the eyebrows is to protect the eye against harmful effects from the outside by surrounding the eye. In addition to their protective function, eyebrows play an important role in the symmetry of the face. It is also the main determinant in the formation of the characteristic appearance and mimics of people. Shedding or thinning of the eyebrows is extremely noticeable in social life and can lead to serious aesthetic concerns and loss of self-confidence. Eyebrow transplantation can be a permanent solution to all problems.
Frequently asked questions about spills, the discomfort caused by the awareness of the environment, the need to constantly camouflage with permanent make-up and crayons are annoying.
Causes of eyebrow loss
Physical trauma - accident, burn etc.
Systemic or local diseases - hypothyroidism etc.
Congenital developmental disorder - not congenital
Trichotillomania – pulling of the eyebrows through tic development
Surgical and medical treatments - radiotherapy, chemotherapy, surgical excisions
In addition to all these medical reasons, the most common cause of eyebrow loss is eyebrow removal to change the shape of the eyebrows. After repeated eyebrow removals, eyebrow roots are damaged, thinned and shed over time. This situation is also known as frown.
Eyebrow transplantation can be performed in those who have reduced eyebrows due to medical reasons or incorrect removal, those with thin eyebrows and those who are not satisfied with their eyebrow shape.
Unfortunately, it is not possible to remove thinned or shed eyebrows with cosmetics or drug treatments.
Unfortunately, there is no permanent alternative to eyebrow loss other than eyebrow transplantation (transplant).
Eyebrow transplantation is the transplantation of hair follicles with non-shedding genetics from the back of the head, from the area between the two ears, under the skin to the area where we want to form eyebrows. The transplanted eyebrows do not fall out, the result is permanent.
In the pre-operative interview, the shape of the newly formed eyebrow is decided together with the person to be transplanted. The surgical procedure is performed with local anesthesia. Pre-procedure preparations are completed and local anesthesia is applied to the area where the eyebrows will be removed and planted. The person is awake. It takes 1-2 hours and it is possible to return to daily life immediately after the procedure. There are no bathroom restrictions. After the procedure, redness and crusting occur for a few days. The transplanted area heals in 5-7 days. Eyebrow regrowth is a gradual process. In the first month, the eyebrows begin to grow, and the complete extension of the transplanted eyebrows is completed in 6-8 months.
This operation is actually a kind of hair transplant. They are hair follicles used in eyebrow transplantation. It is ensured that the hair follicles planted in place of the eyebrows replace the eyebrows.
In eyebrow transplantation, hair follicles can be taken with two methods, as in hair transplantation. One is FUT and the other is FUE.
Eyebrow transplants performed with the FUT method are generally not preferred. It requires an incision at the back of the head and stitches. The most important disadvantage is that it leaves a permanent mark, even if it is thin.
In FUE eyebrow transplantation, hair follicles are taken one by one from the back of the head without an incision or stitches. There is no permanent scar after the procedure.
Hair follicles are used in FUE eyebrow transplantation. They are taken from the back of the head, between the two ears, close to the nape. There are especially single-stranded, delicate hair follicles in this region, and single-stranded hair follicles are necessary for a natural eyebrow transplant result.
Eyebrows are different from hair in many ways. Eyebrow hairs come out at a much more oblique angle to the skin than hair. Their directions are more steep in the inner parts close to the nose and horizontal towards the end. In addition, eyebrows usually consist of single-stranded roots. Eyebrow transplantation requires much more care and attention than hair transplantation. The first thing to do is to choose an experienced plastic surgeon and hair transplant clinic who are competent to do this job.
In eyebrow transplantation, especially single-stranded hair follicles should be selected. This is possible with FUE eyebrow transplantation. The single-stranded hair follicles taken are placed in micro channels that are carefully opened on the skin so that no scars are left. While opening the channels, the previously determined eyebrow shape is adhered to. Particular attention should be paid to the directions of the existing eyebrows and the angles of their exit from the skin. Thus, the result is natural that the environment cannot notice.
The number of roots to be planted varies according to the condition of the fallen eyebrows and the area desired by the patient. An average of 500-700 roots are planted.
How to plan an aesthetic eyebrow shape has been clearly determined in plastic surgery practice. However, during the eyebrow transplantation operation, the opinion of the patients is definitely taken, a shape suitable for the needs of the patients is determined and the eyebrow transplantation is performed by adhering to this shape. It is always wise to make it slightly thicker than planned. Thick eyebrows can be easily taken and brought into the desired shape.
Yeah. Eyebrow transplantation is not performed only in people who experience eyebrow thinning or shedding. It is possible to thicken eyebrows with eyebrow transplantation in people who find their eyebrows thin and want to change their shape. Permanent make-up is not an alternative to eyebrow transplantation because it does not look natural. The result of eyebrow transplantation is completely natural.
Thick eyebrows can be suitable for the facial proportions of the patients in terms of aesthetics, and can be preferred only because it is fashionable. In addition, eyebrow thickening can be done in people who have thinned eyebrows as a result of excessive intake. With eyebrow thickening, the eyebrows are lifted a little, giving the face a more energetic appearance.
Eyebrow thickening is a flexible procedure. If the thickened eyebrows are found to be too thick, they can be taken to the desired thickness. It is not possible to thicken the thin eyebrow, but it is possible to thin the thick eyebrow.
Transplanted eyebrows grow much faster than normal. For this reason, you should take care of your new eyebrows on a weekly basis and cut your long eyebrows. After a certain period of time, the elongation rate decreases.
Yes, the hairs that grow after eyebrow transplantation are permanent. Eyebrow hairs continue to grow for life and are no different from your own eyebrows.
Beard&Mustache Transplantation
In order to appear in an aesthetic integrity in men, the presence and shape of the beard and mustache is at least as important as the presence of the hair. Even if the beard and mustache is not left, its presence is the symbol of masculinity. For men, it means self-confidence and aesthetic appearance.
There are many reasons why men don’t have a beard. The vast majority are hereditary. Apart from this, laser epilation, surgical interventions, burns or accidents may be the causes of beard loss. Beard and mustache loss can also occur in some autoimmune diseases and low testosterone (male hormone). When an organic disease is suspected, testosterone hormone levels should be checked.
As in hair loss, it is not possible to grow a new beard with cosmetics and medical treatments in beard and mustache loss. Beard transplantation is a permanent solution to beard and mustache loss.
Beard and mustache transplantation is increasing rapidly in the Middle East region, where beard and mustache is especially important, such as our country. The biggest reason for this is the increased awareness about the feasibility of beard and mustache transplantation, the emergence of new transplantation methods such as FUE hair transplantation, natural-looking hair transplantation with advancing technology.
